Job summary

Breaking Ground seeks a Clinical Coordinator, Transitional Housing to lead clinical operations in New York City. You will supervise a team of Case Managers and ensure high-quality care, aligned with Housing First and Harm Reduction models.

Responsibilities include developing service plans, documenting progress, and coordinating with outreach teams to support housing placement and prevent homelessness. LMSW is required with at least 3 years of related experience.

Qualifications

• NYS Licensed Masters Social Worker (LMSW) required.
• Minimum 3 years of experience with related populations; supervisory and administrative experience preferred.
• Thorough clinical understanding of Housing First, Harm Reduction, CPI, and motivational interviewing.
• Experience with crisis intervention, risk assessments and incident management.
• Ability to collaborate with a wide range of stakeholders and work in a fast-paced environment.
• Proficiency with Word, Excel, Outlook, and Microsoft Teams; familiarity with databases.

Responsibilities

• Supervise a team of Case Managers.
• Provide regular, structured supervision meetings for direct reports.
• Promote Housing First, Harm Reduction, CPI and trauma-informed practices.
• Develop and implement individual service plans; document client contact and progress.
• Interview and assess potential clients; coordinate care with multiple providers.
• Offer clinical direction and leadership to the program; troubleshoot and strategize solutions.
• Support staff and residents toward housing placement and engagement.
• Oversee documentation of client progress and maintain regulatory and agency standards.
