Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing—affordable housing paired with services designed to help people maintain their homes long-term—is recognized as a proven and cost-effective solution to chronic homelessness.

But we didn't stop at providing housing. Our programs and services help people experiencing street homelessness—especially those who have been on the streets the longest—to come indoors. Through our Street to Home outreach and transitional housing resources, we help people secure and maintain permanent housing.

Position Overview: Reporting to the Assistant Director, Programs, the Clinical Coordinator for Safe Options Support (SOS) supervises Case Managers and Consumer Advocates serving Staten Island. The Clinical Coordinator manages a small caseload and ensures the use of Harm Reduction and Housing First strategies, along with other best-practice clinical interventions to provide high-quality care. The role includes maintaining client records in compliance with regulatory and agency standards.

The Clinical Coordinator primarily works during business hours Monday-Friday, with off-hours availability for emergencies or special projects.

Essential Duties and Responsibilities:
  • Supervise a team of Case Managers and Consumer Advocates.
  • Conduct regular supervision meetings with direct reports.
  • Promote effective case management strategies using Housing First, Harm Reduction, CPI, Motivational Interviewing, and trauma-informed care models.
  • Develop and implement individual service plans; ensure detailed and timely documentation of client contacts and progress.
  • Interview and assess potential clients.
  • Provide clinical direction and leadership, troubleshoot, and strategize around client engagement and challenging behaviors.
  • Foster a culture emphasizing housing placement as a success metric, ensuring clients are ready for housing.
  • Oversee development and implementation of service plans and documentation.
  • Assess and evaluate client functioning.
  • Supervise case conferences.
  • Coordinate care with multiple service providers, including outreach teams and housing providers.
  • Support staff in managing difficult situations and de-escalating conflicts.
  • Perform other duties as assigned.
Minimum Qualifications:
  • NYS Licensed Masters Social Worker (LMSW); must obtain LMSW within six months of employment.
  • At least 3 years of experience with related populations; supervisory experience preferred.
  • Strong clinical understanding of Housing First, Motivational Interviewing, Harm Reduction, behavioral and medical health systems.
  • Experience in crisis intervention, risk assessments, and incident management.
  • Ability to work effectively with diverse stakeholders.
  • Proven success in fast-paced environments with multiple priorities.
  • Ability to delegate, motivate staff, and achieve goals.
  • Excellent verbal and written communication skills.
  • Valid NY, NJ, or CT driver’s license with a good driving record.
  • Proficiency with Microsoft Office Suite; familiarity with Access databases and willingness to learn new programs.
