The Clinical Coordinator of Palliative Care works under the direction of the Senior Director of Nursing and performs activities in accordance with the mission and vision of the organization and the philosophy of the department of nursing. The coordinator works collaboratively with other members of the interdisciplinary team; functioning as a registered nurse, team leader and central coordinator for clinical services rendered to our palliative care patients. The clinical coordinator is a patient and family advocate, facilitating services according to the patient’s wishes. The coordinator participates in the patient’s rounds, leads weekly team meetings, works with the Medical Director and Attending Physicians on the Palliative Care Service Team to plan monthly program meetings and is available as a resource to staff. The Palliative Care Coordinator assumes an active role in the Graduate Medical Education Program. The coordinator is responsible for maintaining information pertinent for quality assurance purposes and program development. Promotes cooperation, a positive attitude, and creates a safe environment in which the patient and family are comfortable and supported in sharing information. Responsible for knowing and adhering to established hospital policies and procedures, including all aspects of safety.
As part of employment at Southampton Hospital, there may be occasions when travel to other locations is needed to support operational needs.
