HopeHealth, Inc. in Wakefield, RI, is seeking a Clinical Coordinator - LPN to support field staff by coordinating patient care and staff schedules. You will report to the Clinical management team and the Chief Clinical Officer, with hours 7am-3:30pm and potential coverage in Providence or Attleboro.

Ideal candidates are RI and MA licensed LPNs with 2+ years of clinical experience, strong assessment and communication skills, and a commitment to patient-centered care and HopeHealth values.

• Minimum of 2 years clinical experience in healthcare, home care, hospital or hospice.

Responsibilities

• Coordinate patient care with clinical staff and manage staff visit scheduling.
• Support patients, families, and clinicians.
• Utilize EMR and reports to monitor clinical, quality and financial metrics.
• Ensure field staff visits meet regulatory timepoints.
• Follow up with patients and facilities as directed by the Patient Clinical Manager.
• Maintain professional development through ongoing training.

About This Hospice Clinical Coordinator Position

TheHospice Clinical Coordinator provides support to the clinical field staff by coordinating patient care, provides support in scheduling of patient visits and staff work schedules , and is a resource to the clinical manager, clinicians and patients. The Hospice Clinical Coordinator works under the direction of the Clinical management team and reports to the Chief Clinical Officer.

This is a full time 7am-3:30pm and is located in the Wakefield RI location primarily but may need to cover Providence or Attleboro if needed for coverage.

What will you do as a Hospice Clinical Coordinator?

• Works closely with Patient Clinical Manager and Hospice Service Coordinators.
• Supports patients, families, and clinicians.
• Scheduling of staff visits per the patient care plan as needed.
• Utilizes reports and EMR features to monitor, track and improve clinical, quality and financial performance metrics
• Monitors field staff caseloads and ensure field staff scheduling visits meet regulatory timepoint requirements.
• Follow up with patients/families and facilities as directed by the Patient Clinical Manager.
• Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through participation in continuing education and in-service classes. Attends all mandatory in-services.
• Consistently apply the organizations values in all aspects of work, complying with established Hope Health mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance
• Other duties as assigned
