Job summary

HopeHealth, Inc. invites qualified LPNs to join our Rhode Island team providing compassionate home-based care. The role covers home visits and care coordination with a nurse case manager and physician, focusing on comfort and quality of life for patients with serious illness.

We offer a part-time schedule (32 hours per week, 8:00am-4:30pm, five days, rotating weekends) and a supportive environment. Prior hospice or oncology experience is preferred, with continued education and certifications

Qualifications

  • Current MA Nursing License
  • Must have well developed nursing assessment and clinical skills.
  • Two years hospice or oncology preferred
  • LPN required
  • Hospice/Palliative care nursing certification preferred

Responsibilities

  • Conducts assessments for home and nursing home patients. Assesses patient and family status based on the physiological, psychosocial, spiritual, and emotional needs presented.
  • Implements the plan of care under the direction of the nurse case manager and attending physician.
  • Instructs the caregiver in the care of the terminally ill based on the individual personal differences, educational background and previous experience.
  • Demonstrates skill in interpersonal relationships with Hospice patient, families, nursing home staff, co-workers and community support services.
  • Maintains accurate records in designated manner to record observations, problems and nursing actions which reflect the care given to Hospice patients / families at time of visits

About This Licensed Practical Nurse (LPN) Position

HopeHealth is seeking an experienced Licensed Practical Nurse (LPN) for its Rhode Island team.

HopeHealth is a fast-growth and innovative not-for profit healthcare organization, focused on caring for people with serious illness.

We deliver a unique array of medical care, care management and support services throughout Southern Rhode Island

This is a part-time, 32 hour per week position, with 8:00am-4:30pm hours, 5 days a week. There is a rotating weekend commitment.

What will you as a Licensed Practical Nurse (LPN)?
