Job summary

HopeHealth, Inc. is seeking a Hospice Clinical Coordinator to support clinical field staff by coordinating patient care and scheduling visits. The role reports to the Chief Clinical Officer and is primarily based in Wakefield, RI, with coverage possible in Providence or Attleboro as needed.

This full-time position requires LPN or Therapy Assistant licensure in RI/MA, at least two years of clinical experience, and strong assessment and communication skills.]

Responsibilities

• Coordinate patient care in collaboration with the Patient Clinical Manager.
• Schedule staff visits per the patient care plan and monitor visits.
• Perform chart audits and use EMR reports to monitor quality metrics.
• Monitor field staff caseloads and ensure regulatory timepoint requirements.
• Follow up with patients, families, and facilities as directed.
• Maintain professional growth and participate in continuing education.

About This Hospice Clinical Coordinator Position

Hospice Clinical Coordinator provides support to the clinical field staff by coordinating patient care, provides support in scheduling of patient visits and staff work schedules , and is a resource to the clinical manager, clinicians and patients. The Hospice Clinical Coordinator works under the direction of the Clinical management team and reports to the Chief Clinical Officer. This is a full time 7am-3:30pm and is located in the Wakefield RI location primarily but may need to cover Providence or Attleboro if needed for coverage.

What will you do as a Hospice Clinical Coordinator?

• Works closely with Patient Clinical Manager and Hospice Service Coordinators
• Supports patients, families, and clinicians
• Scheduling of staff visits per the patient care plan as needed
• Performs chart audits
• Utilizes reports and EMR features to monitor, track and improve clinical, quality and financial performance metrics
• Monitors field staff caseloads and ensure field staff scheduling visits meet regulatory timepoint requirements
• Follow up with patients/families and facilities as directed by the Patient Clinical Manager
• Assumes responsibility for personal growth and development
• Maintains and upgrades professional knowledge and practice skills through participation in continuing education and in-service classes
• Attends all mandatory in-services
• Consistently apply the organizations values in all aspects of work, complying with established Hope Health mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance
• Other duties as assigned
