Job summary

Columbia University Irving Medical Center seeks a Clinical Coordinator III (RN) to join the outpatient practice of the Center For Advanced Cardiac Care (CACC), LVAD and Cardiac Transplantation program. You will coordinate patient care, educate patients and caregivers, and support clinic operations under the guidance of the Chief Nurse Practitioner and Medical Directors.

Qualifications

• BSN required or equivalent combination of education and experience.
• Minimum of four (4) years of nursing experience.
• Current New York State RN license.
• Must be eligible for Basic Life Support (BLS) certification.
• Epic EMR experience preferred; ACLS certification preferred.

Responsibilities

• Provide comprehensive patient care coordination and patient/caregiver education.
• Support the ongoing clinical operations of the CACC Advanced Heart Failure, LVAD, and Cardiac Transplant Program.
• Coordinate diagnostic testing, outpatient clinic follow-up, and review of diagnostic reports.
• Participate in day-to-day management of patients and support the program's strategic vision.
• Provide nursing care in outpatient settings in collaboration with APPs and physicians.
• Complete accurate documentation in the Epic EMR.
• Provide remote patient monitoring and assessment with supervising providers.
