Your Health Organization in Ellijay is seeking a Clinical Coordinator to support all clinical needs for our patients. The role emphasizes chart prep, coordination of care, and successful patient-provider communication in a fast-paced, value-based setting.

Qualifications

• Must be a CMA or licensed nurse (LPN or RN).
• Licensed in the state of employment; certification/license in good standing.
• Minimum of 1 year experience in a healthcare setting preferred.
• Must hold CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all documentation, test results, and relevant information are accessible for the provider.
• Utilize Navina to ensure these are added, completed, or updated.
• Maintain accurate up-to-date patient records including demographics and insurance details.
• Ensure confidentiality and adhere to privacy regulations when handling patient information.
• Verify active consent forms and alignment paperwork are completed and in the chart.
• Reconcile medications.
• Schedule and coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, in person to gather information and provide pre-appointment instructions.
• Appropriately document and log care management activities.

Areas of Responsibility

• Review and organize patient medical records in preparation for visits

• Collaborate with care team to schedule and coordinator patient appointments with the Companys providers and other members of the care team
• Interact with patients via email patient portal telephone text in person etc to gather necessary information schedule and confirm appointments in advance and provide pre appointment instructions
• Communication methods such as phone email AthenaText Microsoft Teams etc will be utilized
• Appropriately and accurately document and log care management activities

Qualifications

• Must be a Certified Medical Assistant CMA or licensed nurse ie LPN or RN
• Must be licensed in the state of employment Certificationlicense must be in good standing with the appropriate board
• A minimum of one 1 year experience in a healthcare setting preferred
• Must hold and maintain current CPR certificate
• High school diploma or equivalent required
• Should demonstrate strong interpersonal and communication skills under all conditions and circumstances
• Ability to foster a cooperative work environment
• Team player with ability to manage multiple responsibilities and demonstrate sound judgment
• Ability to read and communicate effectively
• Strong written and verbal communication skills
• Basic computer knowledge
