Job summary

Your Health in Sumter, SC seeks a Clinical Coordinator to ensure seamless patient care coordination in our primary care setting. You will prepare charts, schedule visits, log care management activities, and support providers and patients daily.

We offer a stable full-time position with standard hours (8:00 AM–5:00 PM), competitive pay, and benefits including 401K matching and PTO. Prior CMA or nursing licensure is required to join our care team.

Qualifications

• Must hold CMA certification or be licensed as a nurse in the state of employment.
• Minimum one year of healthcare experience preferred.
• CPR certification required and maintained in good standing.

Responsibilities

• Review and organize patient records prior to visits.
• Prepare documentation, test results, and diagnoses for providers.
• Coordinate scheduling of patient appointments with care team.
• Log and manage care management activities and communications.
• Maintain accurate patient demographics and contact details.
• Ensure confidentiality and privacy compliance at all times.
