Crossroads Community SOURCE is currently offering an exciting opportunity for the right person to fill our Clinical Case Manager, RN/LPN position!

Our Case Managers work to improve the health outcomes of persons with chronic health conditions, by linking primary medical care with home and community-based services.

Location: Perry. GA

Job Responsibilities :

• Identify, plan, and coordinate community-based services for members Responsible to contact members and caregivers
• Conducts in-home visits and assessments, initial and annual (No direct/hands-on clinical duties)
• Maintains member records and resolves variances
• Provides teaching and education to SOURCE members to promote well-being and health along with the ability to be managed at home
• Maintains privacy and confidentiality of members’ protected health information

Qualifications:

• Experience in long-term care, social work, case management or community-based services
• Good communication and customer service skills
• Excellent organizational and decision-making skills
• Current Georgia RN or LPN license in good standing
• Must have a valid driver’s license, dependable transportation and able to drive in all areas of coverage
• CCSP / SOURCE experience is strongly preferred
• Harmony Case Management System experience is strongly preferred

Pay Range: $45,000 - $55,000 annually dependent on licensure and experience
