JOB SUMMARY

The Registered Nurse Care Team Leader (Case Manager) provides professional nursing services in accordance with physicians’ orders or plan of care to patients age 18 and older for a wide range of diagnoses. The RN, CTL is responsible for determining the nursing needs for an assigned group of patients/residents, initiating the plan, and evaluating the plan's effectiveness.

ESSENTIAL JOB FUNCTIONS
  • Effectively provides comfort and well-being of patients by assessing conditions, processing physicians’ orders, providing treatments, and administering prescribed drugs.
  • Coordinates nursing care for patients and educates patients/residents and family about the care.
  • Delegates appropriate aspects of nursing care to staff, including evaluating, monitoring, and assuring delivery of care.
  • Documents patients/residents progress toward goals and other observations accurately and timely.
  • Reports patients/residents changes, needs, and/or requirements to appropriate staff.
  • Adheres to all safety regulations and requirements.
  • Handles stress well and maintains acceptable standards of workplace conduct.
ESSENTIAL QUALIFICATIONS

Education: Associate's degree in professional nursing (ADN)

Licenses/Certifications:

  • Must be a Registered Nurse by both Missouri and Kansas State Board of Nursing.
  • Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
  • Valid driver’s license and proof of insurance required.

Knowledge and Skills:

  • Basic knowledge of Microsoft operating system and Microsoft Word, Excel, and Outlook.
  • Ability to manage multiple tasks and responsibilities simultaneously.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, and technical procedures.
  • Ability to write reports and business correspondence.
  • Ability to effectively present information and respond to questions from groups.
  • Ability to perform basic mathematical calculations.
  • Ability to define problems, collect data, and draw valid conclusions.
SPECIAL WORKING CONDITIONS
  • Subject to both inside and outside environmental conditions.
  • Exposed to infectious diseases.
  • Required to drive from one location to another.
  • May have “on-call” responsibilities.
  • May assist other departments in emergencies.
PHYSICAL AND MENTAL DEMANDS
  • Exerts up to 50 lbs of force occasionally, and/or up to 20 lbs of force frequently.
  • Close visual acuity required for data preparation, extensive reading, and observations.
  • Required to hear sounds at normal speaking levels and communicate effectively.
  • Requires sustained periods of standing and walking.
SUPERVISION RECEIVED
  • Receives daily supervision from Clinical Manager.
SUPERVISION GIVEN

May provide direction to caregivers on a daily basis.
