The ideal candidate holds a Florida RN license, has 3+ years of clinical experience, and 1+ year in case management. A strong focus on patient experience, documentation, and EMR proficiency is required.

The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in all program areas.
