$32.18 - $33.87 Per Hour

Job Summary and Purpose:

The Clinical Case Management Team provides clients with high acuity and co-morbidities individualized care through service linkages, counseling, treatment planning, and crisis intervention to prepare individuals for success. The Clinical Case Manager II offers Intensive Case Management to clients with multiple psychological co-morbidities for successful societal re-entry.

Duties and Responsibilities:
  • Handle caseloads of higher acuity clients.
  • Conduct comprehensive screenings and assessments to gather functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
  • Assess barriers and develop case plans.
  • Coordinate planning with clients to meet needs and provide support systems.
  • Coordinate case conferences, monitor progress, and assist clients in meeting goals.
  • Advocate for clients with judicial, community, social, and administrative bodies.
  • Network with referral agencies, community resources, and family supports.
  • Participate in outreach events.
  • Perform other tasks to meet program goals.
Qualifications:
Requirements:
  • Pass fingerprint clearance, background check, and employment verifications.
  • Current TB screening and immunization records.
  • Copy of current professional license (if applicable).
  • Current BLS certification.
  • Valid driver’s license.
  • Complete all required trainings within 30 days of hire.
Education:
  • Master's degree in social work, psychology, or counseling from an accredited institution.
  • Registered as an Associate Clinical Social Worker (ASW) or Associate Marriage and Family Therapist (AMFT).
Experience:
  • At least 2 years of human services experience with a focus on complex clinical cases, including homelessness, addictions, underserved populations, or prison re-entry.
  • Minimum of 1000 supervised hours toward licensure (LCSW or LMFT).
Preferred Qualifications:
  • 3 years of clinical experience in relevant program areas.
