Qualifications

• Bachelor's degree in social work, psychology, or related behavioral science.
• Master's degree in social work or related field preferred.
• Knowledge of recovery-focused practices and harm-reduction approaches.
• Ability to work effectively with diverse populations and teams.

Responsabilités

• Provide ongoing case management to adults with complex behavioral health needs.
• Develop care and housing retention plans with multidisciplinary teams.
• Engage clients, conduct psychosocial assessments, and coordinate services.
• Travel locally for outreach and home visits; document services promptly.
