Job summary

The People Concern’s Santa Monica HMST team seeks a master’s level clinical case manager for field-based services delivering outreach to chronically homeless individuals with complex needs.

You will work 100% onsite on a multidisciplinary street outreach team, develop individualized service plans, enroll clients in public benefits, and connect them with medical and behavioral health resources in Santa Monica.

Qualifications

• Master’s degree in a mental health field (MSW, MFT, PhD, PsyD).
• Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
• Demonstrated knowledge of clinical case management.
• Experience with homeless populations with co-occurring mental health, substance use, or medical issues.
• Excellent writing and charting skills.
• Knowledge of community resources.
• Able to maintain relationships with Santa Monica partners (police, firefighters, city staff).
• Ability to self-motivate, work independently, and communicate effectively.
• Able to work as part of an interdisciplinary team.

Responsibilities

• Provide field-based services, meeting clients where they are to eliminate barriers to services.
• Assess mental health service needs of clients.
• Engage clients in developing mutually agreed-upon goals documented in each client’s service plan.
• Provide risk assessment and clinical evaluations for gravely disabled clients.
• Link clients with medical, mental health, substance abuse, and vocational resources.
• Assist clients in obtaining benefits (SSI) and housing applications.
• Provide opportunities to build a supportive peer community.
• Collaborate with County resources to assist clients with severe mental illness/addiction.
• Complete all charting in a timely manner.
• Collaborate with police and hospital staff to reduce ED visits.
• Work on an interdisciplinary team with Program Manager oversight.
• Other duties as assigned.

Summary:

The People Concern’s Santa Monica Homeless Multidisciplinary Street Team (HMST) is seeking a master’s level clinical case manager on this multidisciplinary street outreach team. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of HMST is to target the highest utilizers of the City of Santa Monica’s emergency resources (police, fire, and hospitals). The skilled interdisciplinary team uses best practices for engaging chronically homeless individuals with co-occurring mental health, substance use, and/or medical issues. A low client-to-staff ratios is used to maximize effectiveness. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment.

Essential Duties and Responsibilities:

• Provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services.
• Assess mental health service needs of clients.
• Engage clients in developing mutually agreed-upon goals documented in each client’s confidential service plan.
• Provide risk assessment and clinical evaluations for client’s who are potentially gravely disabled and in need of hospitalization and referral to higher levels of care.
• Link clients with appropriate medical, mental health, substance abuse, and vocational resources.
• Assist clients in obtaining all benefits to which they are entitled, including applying for and advocating for SSI benefits.
• Provide opportunities for clients to build a supportive peer community.
• Build relationships with County mental health and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction.
• Complete all charting in a timely manner.
• Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits by targeted population.
• Work effectively on an interdisciplinary team and collaborate with Program Manager to ensure high level of clinical oversight.
• Other duties as assigned.

• Ability to self-motivate, work independently, and communicate effectively.
• Able to work collaboratively as part of an interdisciplinary team

• BBS licensure

Work Environment:

• Combination of field and office environment. Field consists of street, Client's homes and facilities such as hospitals, clinics, DMV, DPSS, etc.
• Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
• On occasion walk or drive to different local sites
• Will necessitate working in busy and loud environments
• Will be exposed to elements like cold, heat, dust, noise and odor
• May need to bend, stoop, twist, and sit throughout the day
