Job summary

The People Concern's Santa Monica HMST is seeking a master’s level clinical case manager for field-based services with chronically homeless individuals facing co-occurring mental health and/or medical issues.

You will work on an interdisciplinary team, develop individualized service plans, help clients enroll in public benefits, and coordinate with local resources to reduce hospital and ED utilization.

Qualifications

• Master’s degree in a mental health field (MSW, MFT, PhD, PsyD).
• Demonstrated knowledge of clinical case management.
• Experience with populations served (serious mental illness, substance use, physical health issues).
• Excellent writing and charting skills.
• Knowledge of community resources.

Responsibilities

• Provide field-based services, meeting clients where they are to eliminate barriers to services.
• Assess mental health service needs of clients.
• Engage clients in developing service plans with mutually agreed goals.
• Provide risk assessment and clinical evaluations for gravely disabled clients.
• Link clients to medical, mental health, substance use, and vocational resources.
• Assist clients in obtaining benefits including SSI.
• Provide opportunities for clients to build a supportive peer community.
• Build relationships with county resources to improve collaboration.
• Complete charting in a timely manner.
• Collaborate with police and hospital staff to reduce ED visits.
• Work with an interdisciplinary team and Program Manager for clinical oversight.

Job Description Summary

The People Concern's Santa Monica Homeless Multidisciplinary Street Team (HMST) is seeking a master's level clinical case manager on this multidisciplinary street outreach team. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of HMST is to target the highest utilizers of the City of Santa Monica's emergency resources (police, fire, and hospitals). The skilled interdisciplinary team uses best practices for engaging chronically homeless individuals with co-occurring mental health, substance use, and/or medical issues. A low client‑to‑staff ratio is used to maximize effectiveness. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (General Relief, SSI/SSDI, Medi‑Cal), submit housing applications, and access appropriate medical and behavioral health treatment.

Essential Duties and Responsibilities

• Provide field‑based services, meeting currently homeless clients where they are to eliminate barriers to services.
• Assess mental health service needs of clients.
• Engage clients in developing mutually agreed‑upon goals documented in each client’s confidential service plan.
• Provide risk assessment and clinical evaluations for clients who are potentially gravely disabled and in need of hospitalization and referral to higher levels of care.
• Link clients with appropriate medical, mental health, substance abuse, and vocational resources.
• Assist clients in obtaining all benefits to which they are entitled, including applying for and advocating for SSI benefits.
• Provide opportunities for clients to build a supportive peer community.
• Build relationships with county mental health and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction.
• Complete all charting in a timely manner.
• Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits by the targeted population.
• Work effectively on an interdisciplinary team and collaborate with Program Manager to ensure a high level of clinical oversight.
• Other duties as assigned.

Qualifications

• Master’s degree in a mental health field (MSW, MFT, PhD, PsyD)
• Demonstrated knowledge of clinical case management.
• Experience in dealing with issues faced by the population served (serious mental illness, substance abuse, and physical health problems).
• Excellent writing and charting skills.
• Knowledge of community resources.
• Able to maintain an effective working relationship with Santa Monica community partners (including police, firefighters, city staff, etc.) and maintain and build professional rapport.
• Ability to self‑motivate, work independently, and communicate effectively.
• Able to work collaboratively as part of an interdisciplinary team.
• Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.

Preferred Qualifications

• BBS licensure.

Work Environment

HMST works in the field and in the office. Field consists of street, client homes, and facilities such as hospitals, clinics, DMV, DPSS, etc.
