CVS Health is seeking a licensed behavioral health clinician for a 100% remote Clinical Case Manager role, serving members nationwide. You’ll conduct assessments, develop care plans, and coordinate with providers to support whole-person health.

Qualifications

  • Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field.
  • Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC).
  • 3+ years of post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar setting.
  • 3+ years of experience treating mental health and substance use disorders.
  • 3+ years of experience utilizing Motivational Interviewing techniques.
  • Case Management experience.
  • Ability to talk on the phone and document in clinical systems.
  • Strong technical proficiency and comfort working in a virtual environment.

Responsibilities

  • Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews.
  • Develop individualized care plans addressing behavioral and physical health needs.
  • Coordinate care with providers, community organizations, and support services.
  • Apply advanced clinical judgment to identify and mitigate risk factors impacting outcomes.
  • Perform crisis intervention for behavioral health or medical crises and arrange referrals.
  • Utilize Motivational Interviewing to engage members and promote behavior change.
  • Coordinate psychosocial wraparound services and connect with community resources.
  • Support cost-effective care through proactive planning and resource utilization.

Clinical Case Manager - Behavioral Health (Licensed)

What You'll Do

WHAT YOU BRING - REQUIRED
  • Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field
  • Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent)
  • 3+ years of post-master's direct clinical experience in a hospital, ambulatory, outpatient, or similar clinical setting
  • 3+ years of experience treating mental health and substance use disorders
  • 3+ years of experience utilizing Motivational Interviewing techniques
  • Case Management experience
  • Ability to simultaneously talk on the phone and document in clinical systems
  • Strong technical proficiency and comfort working in a virtual environment

Pay Range

The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
