Job summary

AccordCare Home Health in Maitland, FL seeks an RN Clinical Manager to lead care coordination and ensure high-quality patient outcomes. This full-time, in-office role (8:30 AM - 5:00 PM) includes limited field case management and requires strong clinical leadership.

You will assess patients, coordinate care with the healthcare team, communicate with referral sources, supervise field staff, and ensure the plan of treatment and authorizations are followed for Medicare home health patients.

Qualifications

• Current Florida RN license required.
• Minimum 1 year of Medicare Home Health case management experience.
• Minimum 1 year of Medicare OASIS experience.
• Must have Medicare home health experience.
• Excellent time management and organizational skills.
• Strong communication and customer service abilities.

Responsibilities

• Perform case management, oversight, and coordination of care for all skilled disciplines.
• Coordinate patient admissions with other members of the healthcare team.
• Provide nursing care and treatment of home care patients per plan of treatment and approved authorizations.

Starting at $75,000/year | Total Compensation Package Based on Experience AccordCare Home Health, a licensed Medicare-certified agency, is seeking a customer service-driven and detail-oriented RN Clinical Manager to join our Maitland team! This position is full-time, in-office (8:30 AM - 5:00 PM) and includes limited field case management as needed.

The RN Clinical Manager is responsible for assessments, coordination of care, communication with referral sources, and supporting field clinicians to ensure the highest quality of patient care.

• Experience with Kinnser / WellSky Medicare is a plus
• MUST HAVE MEDICARE HOME HEALTH EXPERIENCE
