Job summary

AccordCare Home Health in Maitland is seeking an RN Clinical Manager who is customer-service focused and highly organized. This full-time, in-office role supports field clinicians, conducts assessments, coordinates care, and ensures top-quality patient outcomes.

Responsibilities include case management, admissions coordination, and supervising field staff. Florida RN license and Medicare home health/OASIS experience are required; CPR certification and strong communication skills are essential.

Qualifications

• Florida RN license required with CPR certification.
• Minimum 1 year Medicare Home Health case management experience.
• Minimum 1 year Medicare OASIS experience.
• Excellent time management and organizational skills.
• Strong communication and customer service abilities.
• Experience with Kinnser/WellSky Medicare is a plus.
• Must have Medicare home health experience.

• Coordinate patient admissions with other members of the healthcare team.

Job Details

AccordCare Home Health, a licensed Medicare-certified agency, is seeking a customer service-driven and detail-oriented RN Clinical Manager to join our Maitland team! This position is full-time, in-office (8:30 AM - 5:00 PM) and includes limited field case management as needed. The RN Clinical Manager is responsible for assessments, coordination of care, communication with referral sources, and supporting field clinicians to ensure the highest quality of patient care.
