Job summary

Humana Inc. is seeking a Clinical Care Nurse (RN) for in-clinic support at CenterWell Senior Primary Care. The RN improves patient outcomes through transitions of care, reduces avoidable ED visits, and contributes to Medicare Advantage Stars and quality performance.

The role involves care coordination across multiple clinic sites and requires daily on-site presence. You will engage patients and providers, deliver education on preventive care and chronic disease management, and support CMS

Qualifications

• ADN or BSN required and active RN license.
• 3+ years of clinical nursing experience with transitions of care, quality improvement, or population health.
• Proficient with electronic health records (Athena EMR) and data analytics tools (DataHub, Compass Rose, SalesForce HealthCloud).
• Proficiency in Microsoft Office Suite and basic Life Support training.

Responsibilities

• Identify care gaps using clinical data to improve Stars and HEDIS measures.
• Coordinate timely post-discharge follow-ups per TOC/TCM requirements.
• Provide outreach via phone, telehealth, and in-clinic visits for education and care management.
• Document outreach and outcomes per CMS and organizational standards.
• Collaborate with interdisciplinary teams to implement evidence-based interventions.
