Job summary

Bardavon Health Innovations is seeking a Clinical Care Advocate to support reauthorization workflows by processing physician scripts and signed plans of care from approved network providers, ensuring organized, patient-centered service.

You will maintain case documentation, coordinate visits, obtain additional orders, and communicate with stakeholders to prevent care lapses while optimizing workflows and empathy-driven care.

Qualifications

• Must have strong critical thinking and communication skills.
• Proficiency in Microsoft Office Suite, CRM, Salesforce.
• Knowledge of Raintree and bNOTES.
• Ability to work independently and cross-functionally.

Responsibilities

• Maintain accurate and timely documentation of cases; coordinate visit notes, claims, and upcoming medical visits.
• Follow up on referral cases at the time of reauthorization to determine the plan of care.
• Obtain additional orders for continued care and authorization with required stakeholders.
• Ensure patients do not experience a lapse in care at reauthorization.
• Communicate claim status to stakeholders as needed.
