Job summary

Providence Facey Medical Foundation seeks a Coordinator Clinical Care to deliver direct patient care under physicians or nurses, coordinating care for a primary care patient panel. You will support clinical teams, monitor processes, and target high-acuity patients to improve outcomes.

You will work with the Practice Manager and lead physician to evolve this role to best serve patients while upholding the foundation's values and mission.

Qualifications

• Graduate of school for medical assistants and certificate.
• Diploma/certificate from the military in Medical Specialist Course.
• 2 years Healthcare experience.

Responsibilities

• Administers care coordination activities for primary care patients.
• Monitors care coordination processes and supports clinical teams.
• Identifies high acuity patient population and coordinates care accordingly.
