Full-time; Monday- Friday, 8am-5pm. Some weekends or evenings may be required.

General Description:

The Clinical Care Manager takes a leadership role when it comes to managing and monitoring the care of patients. The Clinical Care Manager is responsible for coordinating each student’s clinical care, creating policy and procedures for clinical care, and maintaining a positive physical clinical environment. This person will connect patients with relevant care professionals, work to educate patients on their conditions, and monitor progress. Reports to the AVP for Wellbeing Services and Resources/Dean of Students.

Essential Functions and Responsibilities:

Patient Care Coordination:

  • Coordinate patient care activities, ensuring seamless transitions and optimal outcomes.
  • Develop and implement comprehensive care plans, addressing individual patient needs.
  • Manage patient schedules, appointments, and follow-up care.
  • Facilitate communication between students and clinical and non clinical staff in Wellbeing Services and Resources.
  • Assess patient needs and provide support and resources.
  • Provide patient education and support regarding treatments, procedures, and medications.
Communication & Interpersonal Skills:
  • Demonstrate excellent communication and interpersonal skills to build rapport with students, faculty, and staff.
  • Effectively communicate complex medical information to students.
  • Collaborate with a multidisciplinary team to ensure coordinated care.
Administrative & Organizational Skills:
  • Maintain accurate and organized patient records.
  • Manage incoming student compliance requirements (e.g., MMR verification and other health documentation).
  • Manage patient inquiries and concerns efficiently.
  • Assist with insurance verification and billing inquiries.
  • Oversee the Wellness Access Plan.
  • Participate in quality improvement initiatives using collaborative problem-solving strategies to ensure complete, accurate, and strength-based documentation for all students served.
Knowledge, Skills, and Abilities:
  • Proficient in using electronic health records ( EHR ) systems.
  • Experience in patient advocacy and case management.
  • Knowledge of relevant healthcare regulations and guidelines.
  • Ability to have licensure as a Social Worker.
  • Experience working with college age students.
  • Strong organizational skills.
  • Ability to work in a fast paced environment with competing priorities.
  • Experience with community based social services resources/services.
  • Ability to work with others through a genuine strengths-based approach, appreciating unique qualities and assets of all involved.
  • Excellent communication skills, knowledge of general clinical administration, and cultural competency for communicating with people of various backgrounds.
Required Qualifications:
  • Bachelor’s degree in an applied behavioral sciences discipline or equivalent.
  • Finalist candidates must satisfactorily complete pre-employment background.
Preferred Qualifications:
  • Case management experience in a higher education context.
  • Masters degree in Behavioral Sciences, or equivalent, accredited degree.
  • Possess or have the ability to obtain a license as a social worker.
  • Preference for a flexible schedule rather than a traditional, fixed, 8am-5pm, Monday through Friday.
  • Ability to offer intensive and strength based therapeutic mental health services and supports to students including: assessing for immediate safety and stabilization needs, treatment plan development, safety and crisis planning, on-going assessment and evaluation of current treatment planning strategies, goals, and outcomes.

Physical Requirements:
  • Sitting for extended periods.
  • Ability to lift 20lbs with assistance.
