Job summary

Journey Nursing Services is seeking a Clinical Care Coordinator (CNA/HCA) to oversee home care caseloads, conduct initial and ongoing assessments, and lead caregivers in the field. The role blends clinical support with leadership, quality assurance, and care coordination to ensure excellent client outcomes.

You will travel within Washington State, support scheduling, documentation, and interdisciplinary communication, and participate in ongoing professional development opportunities.

Qualifications

• Current Washington CNA/HCA certification.
• Current CPR/BLS certification.
• Minimum two years of caregiving experience.
• Strong leadership, organizational, communication, and problem-solving skills.
• Valid Washington State driver’s license and reliable transportation.

Responsibilities

• Manage an assigned caseload of home care clients and perform Start of Care assessments.
• Provide caregiver orientation, supervision, and ongoing education.
• Ensure compliance with agency policies, payer requirements, and WA State regulations.
• Collaborate with scheduling to maintain staffing and coverage for cases.
• Serve as primary liaison among clients, families, caregivers, providers and leadership.
