Zusammenfassung

Phamily, as part of Jaan Health, is seeking a compassionate Clinical Care Lead RN to support patients in a text-based Advanced Primary Care Management (APCM) program. The role delivers proactive care via digital communication, improving health outcomes and patient engagement in a virtual care setting.

Ideal candidates are patient advocates with strong critical thinking, care coordination skills, and experience leading teams in a care management context.

Qualifikationen

• 4+ years in a care management or clinical care coordination role (primary care, chronic care, etc.)
• Previous experience managing a team
• Strong critical thinking and problem-solving skills in a fast-paced environment
• Ability to manage a digital patient panel and prioritize escalations
• Excellent written communication and documentation skills
• Comfort using EMRs, care coordination platforms, and digital messaging tools
• Must have reliable internet and a secure/private environment
• Travel up to 10%, including potential overnight travel

Aufgaben

• Monitor and respond to patient messages and check-ins in a timely, compassionate, and professional manner
• Identify, assess, and triage clinical concerns, SDOH needs, and care coordination barriers
• Develop and update personalized care plans for patients; educate and motivate patients to adhere to the plan
• Oversee clinical quality of the team, analyze data to manage direct reports, and train new staff
• Develop, implement, and revise care coordination workflows to improve efficiency and scalability
• Lead quality improvement initiatives, conduct case reviews and chart audits, and host team meetings
• Identify and troubleshoot issues during new program iterations and escalate to leadership
• Collaborate with CMAs, LPNs/RNs, NPs, and Social Workers for timely escalation
