Ideal candidates are experienced RNs who thrive in virtual care, demonstrate strong care coordination skills, and can manage a team while navigating hybrid work arrangements. Travel up to 10% may be required.

Qualifications

• Active RN license with 4+ years in care management or care coordination.
• Experience leading a team and coordinating patient care.
• Strong written communication and ability to use EMRs and digital tools.
• Willingness to travel up to 10% and work in a hybrid/onsite model.

Responsibilities

• Monitor and respond to patient messages and check-ins promptly.
• Triage clinical concerns and social determinants of health needs.
• Develop and update personalized care plans; educate patients.
• Oversee team clinical quality and train staff.
• Design and revise care coordination workflows for scalability.
• Lead quality improvement initiatives and conduct chart audits.
