Your Health Organization in Savannah, GA is seeking a Clinical Coordinator to support all clinical needs for our patients. The role emphasizes chart preparation, patient scheduling, care management logging, and provider support. The position is full-time, 8:00 AM–5:00 PM, Monday–Friday, in the Savannah office.

Requirements include CMA or licensed nurse credentials, CPR certification, and at least 1 year in a healthcare setting. The role involves travel between offices and facilities as needed.

Qualifications

• Must be CMA or licensed nurse (LPN or RN) in the state of employment and in good standing.
• Strong interpersonal and communication skills.
• Basic computer knowledge.
• Must be able to travel between offices and facilities.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure documentation, test results, and diagnoses are accessible for the provider.
• Verify active consent forms and alignment paperwork in the patient’s chart.
• Reconcile medications.
• Schedule and coordinate patient appointments with the provider and care team.
• Communicate with patients via email, portal, phone, text, and in person to gather information and provide pre-appointment instructions.
• Log and document care management activities.
