Basic Purpose Of The Position

As part of a person-centered care model, this role will be responsible for assisting in the implementation and oversight of all aspects of case management. Using best practice concepts, this role will assist the clinical team in care planning, appropriate utilization of services, and coordination of care. The primary focus remains on providing person-centered care efficiently and ensuring patient satisfaction. This role will collaborate with all team members to achieve exceptional clinical, quality, and financial outcomes.

The main activities include:

  1. Reviewing clinical information and assessment data, making recommendations for efficient and effective utilization of services to achieve positive patient outcomes and organizational metrics.
  2. Analyzing data, investigating outliers, and reporting appropriately.
  3. Participating in daily and weekly case conferences with clinical teams.
  4. Promoting a cooperative, cohesive group process dedicated to quality patient care and optimal outcomes; collaborating across regions.
  5. Participating in Performance Improvement activities within the agency.
  6. Ensuring quality, clinical, financial, and patient satisfaction outcomes.
  7. Managing case activities, including authorization of services, physician identification, and compliance with the plan of care.
  8. Maintaining knowledge of and compliance with federal, state, and accreditation regulations, as well as agency policies and procedures.
  9. Maintaining utilization statistics aligned with national benchmarks and optimizing clinical outcome scores as evidenced by Home Health Compare and similar metrics.
  10. Running daily clinical huddles, participating in IDTs, Lean Daily Management, and daily SBAR with OASIS and revisit nurses.
  11. Coaching and evaluating Licensed Professional Nurses, including personnel evaluations per agency policy.

Qualifications

Licensure: Registered Nurse with a license to practice in Connecticut.

Education: Graduate of an Associate's or diploma program in nursing; NLN-approved BSN preferred.

Experience: One year of nursing experience or in a specialty such as medical/surgery or home care preferred. Computer skills are a plus.
