Position Purpose

This role supports our person-centered care model by assisting in the implementation and oversight of case management activities. Using best practices, you will aid the clinical team in care planning, service utilization, and care coordination. The primary goal is to deliver person-centered care efficiently and ensure high patient satisfaction. Collaboration with team members to achieve excellent clinical, quality, and financial outcomes is essential.

Key Activities
  • Review clinical information and assessment data; recommend efficient utilization of services to achieve positive outcomes and meet organizational metrics.
  • Analyze data, investigate outliers, and report findings appropriately.
  • Participate in daily and weekly case conferences with clinical teams.
  • Promote a cooperative team environment focused on quality patient care and optimal outcomes; collaborate across regions.
  • Engage in Performance Improvement activities within the agency.
  • Ensure quality, clinical, financial, and patient satisfaction outcomes.
  • Manage case activities including service authorization, physician identification, and compliance with care plans.
  • Maintain knowledge of and compliance with applicable federal, state, and accreditation regulations, as well as agency policies.
  • Keep utilization statistics aligned with national benchmarks and optimize clinical outcome scores as reflected in Home Health Compare and similar metrics.
  • Lead daily clinical huddles, participate in IDTs, Lean Daily Management, and daily SBAR meetings with OASIS and revisit nurses.
  • Coach and evaluate Licensed Professional Nurses; complete personnel evaluations per agency policy.
Qualifications
  • Licensure: Registered Nurse licensed to practice in Connecticut.
  • Education: Graduate of an Associate's or diploma nursing program; BSN from an NLN-approved program preferred.
  • Experience: At least one year of nursing experience, preferably in medical/surgical or home care. Computer skills are preferred.
