Job summary

Qualifications

• High School diploma with 1 year of relevant work experience (health care, mental health, customer service, admin).
• Bachelor’s degree in behavioral sciences or related field preferred.
• Certification as Community Health Worker or Family Peer Specialist preferred.
• Valid Minnesota driver’s license required.
• Commitment to diversity, multiculturalism, and inclusion.

Responsibilities

• Support integration of treatment plans and assessments with Fraser clinicians and clients.
• Help individuals access services across mental health, social, educational, health and recreation areas.
• Identify barriers to accessing services and assist in overcoming them.
• Communicate with care team for continuity of care.
• Conduct client appointments during scheduled hours.
• Provide consultation with internal/external providers regarding shared clients.
• Document client data and social determinants of health in Fraser’s EHR.
• Collaborate with providers to ensure access for families.
