POSITION SUMMARY: The Collaborative Care team involves the patient’s Primary Care Provider (PCP), a Psychiatric Consultant, Clinical Behavioral Care Manager (CBCM), and, when available, other mental health providers in the primary care clinic. The CBCM will provide assessment and care management services, including administration of validated rating scales, technology enabled psychosocial interventions, behavioral health care planning in relation to psychiatry health problems, including revision of plans for patients who are not progressing or whose status changes, maintenance of the online member registry, and ongoing collaboration with partner practitioners and our psychiatric care team.
ESSENTIAL FUNCTIONS:
- Support the mental and physical health care of patients for an assigned patient caseload.
- Assess, plan, implement, monitor and evaluate options and services to meet psychosocial health needs of patients.
- Provide brief behavioral health interventions using evidenced-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
- Systematically track treatment response and monitor patient for changes in clinical symptoms.
- Build continuous relationships by telephone or tele-video capabilities.
- Engage patients outside of normal clinic hours as necessary to perform CBCM duties.
- Work with Psychiatric Consultant to support psychotropic medication management as prescribed by medical providers focusing on treatment adherence, monitoring side effects, and effectiveness of treatment.
- Deliver weekly patient care plan reviews with the psychiatric care team and collaborative care team members.
- Facilitate and support regular collaborative case reviews with medical providers by telephone or video.
- Utilize a shared technology platform to track all assigned patients and prioritize patients for case review.
- Identify and escalate patients who may need more intensive or specialized mental health care, as appropriate.
- Respond to communications from team members, patients, and medical providers in a timely fashion.
- Perform other related tasks as assigned.

- Ability to quickly synthesize medical and behavioral health data and formulate effective and evidence-based clinical recommendations.
- Maintain active licensure within state(s) of jurisdiction.
- Demonstrated success managing patient caseload from home environment.
- Experience working within primary care/psychiatry under the oversight of a billing practitioner.
- Strong knowledge of clinical standards of care.
EXPERIENCE / EDUCATION REQUIREMENTS:- Completion of Master of Social Work or equivalent accredited program.
- Current LCSW, LCAS, or other licensed mental health professional license. The license can be at an associate or supervised level, but we are not always able to provide to the necessary supervision.
