Aegis Healthcare is seeking a Clinical Liaison / Clinical Patient Care Navigator in Arizona to connect with patients, families, and referral partners, expanding our continuum of care. You will leverage your clinical background to represent our home health, hospice, and related services while meeting monthly goals.

Candidates should have hospice experience, strong relationship-building and communication skills, and a proactive, self-motivated approach.

Qualifications

• Hospice clinical background required.
• Experience building referral relationships in senior care.
• Knowledge of hospital case management and skilled nursing.
• Excellent written and verbal communication skills.
• Valid driver’s license and reliable transportation.
• AZ Fingerprint Clearance Card preferred; CPR certification beneficial.

Responsibilities

• Represent the organization and build meaningful relationships with patients, families, and referral partners.
• Deliver presentations on Home Health and Hospice care to grow services.
• Collaborate with team to meet monthly goals and ensure high-quality care.
• Expand business opportunities within assigned territory across Atlas Medical, Aegis Home Health, and Aegis Hospice.

About the Role

Compensation: $85,000-$90,000/yearly DOE Status: Full-Time, W2 Employee Schedule: Monday - Friday, 8 AM - 5 PM (Occasional on-call weekends)Location: Arizona (West Valley, East Valley, and Tucson)

As a Clinical Patient Care Navigator, you will have the unique opportunity to utilize your clinical background to connect with patients, families, and referral partners with our exceptional continuum of healthcare services. In this role, you will focus on strategically expanding our business within your assigned territory while strengthening existing partnerships and fostering new connections. You will play a key role in identifying and generating opportunities to grow our services for Atlas Medical, Aegis Home Health, and Aegis Hospice. Your responsibilities will include representing our organization, building meaningful relationships, and delivering impactful presentations on Home Health and Hospice care. Additionally, you will collaborate with our team to achieve monthly goals and ensure the highest standards of service and care. • Have Hospice Clinical background 2+ years of Hospice Referral RelationshipsKnowledge of local Senior Care Industry, Hospital Case Management, and Skilled NursingIntuitive, solution-oriented, and driven to achieve goalsDetail-oriented, flexible, and self-motivatedExcellent oral and written communication skillsValid Driver's License (clean record) & reliable transportationProof of current auto insurance & car registrationValid AZ Fingerprint Clearance Card (we can help with this)Current CPR Certification (we can help with this) & Negative TB Test (we can test)
