Qualifications

  • Experience in durable medical equipment intake and setup.
  • Two-year degree in a healthcare-related field preferred.

Job description

  • Position: Clinical and Liaison Office Specialist
  • Location: Hybrid- 50% at our Milluwake office and 50% at Children's Wisconsin (Milwaukee)
    • Must be willing to travel
  • Hours: 8am-5pm & on call hours

THE POSITION:

Collaborates with the Children’s of Wisconsin Liaison Office (CHW LO) Program as the face of PHS in addition to supporting referring providers and customers with delivery, education, and routine follow up related to low tech respiratory durable medical equipment/supplies and enteral equipment/supplies.

Monitors referrals and supports intake process through setup and initial patient admission to PHS from referring facility (CHW LO Program).

Acts as a liaison between hospital staff, PHS, and others on patients care team to support hospital discharge (CHW LO Program).

Coordinates intakes from Admissions to Clinical to ensure service setup is completed as specified by the referring provider and/or family.

Prepares, delivers and sets up low tech/enteral durable medical equipment, oxygen and/or supplies to patients at the site of care selected by the referral and/or family preference.

Provides education to caregivers; follows up according to PHS policy and patient care plan to ensure safe, effective use of equipment and supplies.

Conducts Home Environmental Assessments per PHS policy.

Completes follow-up visits for low tech respiratory and enteral patients per PHS policy, reporting potentially unsafe conditions to family, leader and referring provider as needed.

Assists patients in resolving equipment and/or supply related problems.

Exchanges equipment due for maintenance or deemed to be malfunctioning.

Sets up and picks up diagnostic studies and ensures delivery to appropriate physician.

Responds to emergency calls (during open office hours or after hours/on-call) with backup support provided by clinicians and leadership.

Completes timely communication and documentation per PHS policy.

REQUIREMENTS:

  • Durable medical equipment (DME) intake experience
  • Must have a valid driver’s license.
  • Two-year degree, preferably in healthcare related field.
  • Ability to use navigation system and follow maps.
  • Experience in Brightree EHR a plus!
