Job summary

Memorial Physician Practices is seeking a Pharmacist - Non-Exempt to coordinate pharmaceutical care in collaboration with an interprofessional team, review orders, and ensure safe, effective medication use for patients.

The role requires a Doctor of Pharmacy degree, a current state license, BCLS certification (ACLS preferred), and proficiency with pharmacy software. The position includes participation in outpatient anticoagulation care, education, and quality improvement activities.

Qualifications

• Doctor of Pharmacy degree required.
• Current license with the state Board of Pharmacy.
• BCLS required; ACLS preferred.
• Excellent English communication and ability to work with minimal supervision.
• Experience with pharmacy software systems.

Responsibilities

• Manages drug therapy protocols as part of an interdisciplinary team.
• Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems.
• Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Documents interventions and outcomes for drug therapy quality improvement.
• Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.
• Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
• Participates in performance improvement and required continuing education.
• Performs other duties as assigned.
