Voyage Healthcare is seeking a Clinic Triage RN for its ambulatory clinic in Crystal, Minnesota. The role covers telephone and in-person triage, nurse visits, immunizations, patient education, care coordination and EMR documentation.

Qualifications

• Two years RN experience preferred; strong communication and assessment skills.

Responsibilities

• Provide telephone and in-person triage using established protocols.

• Administer medications, injections and immunizations.

Voyage Healthcare is seeking a Clinical Support/Triage RN to provide patient-centered nursing care in an ambulatory clinic setting. This role includes telephone and in-person triage, nurse visits, immunizations, patient education, care coordination, and clinical support. The RN collaborates with providers and care teams to ensure safe, timely, and effective patient care while practicing within the Minnesota Nurse Practice Act and established clinical protocols.

Hourly Range:$34.00 - $41.00, based on years of relevant experience.

• Assess patient symptoms and determine appropriate level of care
• Administer medications, injections, and immunizations
• Conduct nurse visits and provide patient education
• Assist with clinical procedures and treatments
• Coordinate patient care and support case management activities

• Respond appropriately to urgent and emergent patient situations
• Document patient assessments, interventions, and communications in the EMR
• Deliver culturally competent, patient-focused care
• Maintain compliance with organizational policies, OSHA standards and professional nursing practices

Qualifications:

• Graduate of an accredited school of nursing program
• Current Minnesota Registered Nurse (RN) license
• Current CPR certification
• Two years of RN experience preferred
• Strong communication, assessment, and critical-thinking skills
• Ability to work independently and collaboratively in a fast-paced clinic environment
