Job summary

Voyage Healthcare MN in Crystal, MN is seeking a Clinical Support/Triage RN to provide patient-centered nursing care in an ambulatory clinic setting. This role includes telephone and in-person triage, nurse visits, immunizations, patient education, care coordination, and clinical support.

The RN collaborates with providers and care teams to ensure safe, timely, and effective patient care while practicing within the Minnesota Nurse Practice Act and established clinical protocols.

Qualifications

• Graduate of an accredited nursing program.
• Minnesota RN license is required.
• CPR certification.
• Two years of RN experience preferred.

Responsibilities

• Provide telephone and in-person triage using established protocols
• Assess patient symptoms and determine appropriate level of care
• Administer medications, injections, and immunizations
• Conduct nurse visits and provide patient education
• Assist with clinical procedures and treatments
• Coordinate patient care and support case management activities
• Process prescription refills according to protocol
• Respond appropriately to urgent and emergent patient situations
• Document patient assessments, interventions, and communications in the EMR
• Deliver culturally competent, patient-focused care
• Maintain compliance with organizational policies, OSHA standards and professional nursing practices

Hourly Range:$34.00 - $41.00, based on years of relevant experience.
