Job summary

Ascension is seeking a registered nurse for our Internal Medicine clinic in Carmel, IN. You will perform rapid clinical assessments and triage patient needs while coordinating care between patients, providers, and specialty teams to optimize clinic flow.

Qualifications

• Licensed RN with Indiana state license required before hire date.
• BLS certification required within 30 days of hire.
• Diploma from an accredited nursing program or equivalent licensure.

Responsibilities

• Perform rapid clinical assessments by phone or in person to triage patient needs.
• Direct patients to appropriate care level to ensure safety and flow.
• Serve as the hub between patients, providers, and specialty teams for fast care decisions.
• Coordinate immediate clinical orders during or before clinic visits.
• Educate patients on symptom management and next steps during transitions of care.
