Job summary

UP Health System - Portage in Michigan is seeking a Clinic RN to deliver direct patient care in a fast-paced, supportive environment dedicated to community health.

You will administer medications, document care, monitor patient conditions, and collaborate with physicians, other nurses, and social work to coordinate transfers, discharges, and family education. A Michigan RN license and ADN are required, with a BSN preferred. BLS and influenza vaccination are required.

Qualifications

• Current Michigan RN license (or obtain within 90 days of hire).
• Associate degree from an accredited nursing school; bachelor’s degree preferred.
• Basic Life Support certification is required.
• Annual influenza vaccination or declination required.

Responsibilities

• Performs plan of care interventions, including medication administration, specimen collection, and clinical treatments.
• Documents patient care provided.
• Monitors, records, and communicates patient condition to care team, physician, nursing home, patient, and family.
• Collaborates across disciplines to coordinate care, including transfers and discharges.
• Evaluates learning needs of patient and family and provides education.
• Advocates for the patient’s physical and emotional well-being.
• Delegates care activities to appropriately qualified staff.
• Performs prior authorizations on tests, procedures, medications as required by insurance.
• Performs other duties as assigned.
