Job summary

Avera is seeking a Registered Nurse to support patient care navigation in the clinic setting and promote effective education, self-management, and timely care delivery. You will develop care management processes and identify high-acuity patients for coordinated care.

The RN will collaborate with clinicians and leadership to optimize outcomes, working closely with Care Coordination and Medical Home teams to report on quality measures. Strong communication and teamwork are essential.

Qualifications

• RN with Board of Nursing certification.
• Active license in the state of practice required.
• Experience in clinic nursing or case management helpful.

Responsibilities
