Job summary

Avera is seeking a Registered Nurse (RN) coordinator at Avera McKennan Hospital in Sioux Falls to manage patient transfers and placement across the system. You will serve as a liaison among nursing, physicians, and facilities to ensure timely and appropriate patient movement.

The role emphasizes collaboration, clinical data collection, and efficient use of healthcare technology to support continuity of care and decision-making. 40 hours/week, primarily days, with potential evenings and weekends.

Qualifications

• Must be able to work the hours specified including days/evenings/weekends
• Active RN license in state of practice upon hire
• Demonstrated communication and collaboration skills with healthcare teams

Responsibilities

• Oversees patient flow from facilities to ensure efficient entry into the hospital
• Prioritizes patient placement across regional sites using policies and guidelines
• Liaises with bed placement coordinators for timely transfers
• Evaluates transfer center processes to achieve outcomes
• Collaborates with physicians and nurses to assess patient placement
• Uses evidence-based findings and healthcare tech in practice
