Qualifications

• Active RN license in the state of practice.
• Ability to coordinate home health/hospice services post-discharge.
• Strong communication and teamwork skills.

Responsibilities

• Coordinate patient's clinical services between hospital and home.
• Educate patients and families about home health/hospice options.
• Obtain physician orders and coordinate post-acute home care plans.
• Communicate with hospital and agency staff to improve continuity of care.
• Maintain knowledge of federal/state regulations and agency policies.
