At Sunrise, our Wellness Nurse is responsible for the overall well-being of our residents.

Responsibilities & Qualifications

We are seeking a PRN/On-Call RN for our Independent Living Health & Wellness Clinic. The clinic is open 7 days a week from 7:30 am to 4 pm, with extended evening hours on Monday & Wednesday until 8 pm.

Minimum 2 years experience in hospital, home health, assisted living, or long-term care setting. Experience in outpatient/ambulatory/physician office preferred.

Responsibilities include:
  1. Identifying ongoing needs and services of residents to promote high-quality resident services.
  2. Ensuring proper follow-up, assessment, and documentation for residents with changes in health care services and status.
  3. Assisting Resident Care Director (RCD) in conducting resident assessments at move-in, 30 days post-move-in, semi-annually, and with any change in status.
  4. Conducting monthly wellness visits for all residents.
  5. Assessing, monitoring, and following up with residents after falls, accidents, or incidents; documenting and reporting as required.
  6. Maintaining communication with residents’ families or responsible parties to meet medical needs.
  7. Contacting attending physicians as necessary or upon family request.
  8. Ensuring weights and vital signs are obtained monthly for each resident.
  9. Maintaining wellness files according to policies and regulations.
  10. Maintaining medical supplies and emergency kits.
  11. Providing clinical support and assistance to team members.
  12. Following infection control practices and promoting standards within the community.
  13. Monitoring medication and treatment profiles to ensure proper administration and documentation.
  14. Participating in quality improvement initiatives and understanding relevant regulations.
Qualifications:
  • Graduate of approved college/school of nursing.
  • Current state RN license.
  • At least one year of experience in relevant healthcare settings.
  • Knowledge of nursing practices, regulations, and assessment skills.
  • Effective communication and organizational skills.
  • Proficiency in computer applications including Microsoft Office and Sunrise systems.
  • Ability to work flexible hours, including weekends and evenings.

Compensation

Competitive pay based on location, skills, experience, and certifications. Salary range: USD $36.05 - USD $48.10 per hour. Overtime eligible. Sunrise is an Equal Opportunity Employer.
