Job summary

Responsibilities include triaging patient concerns by telephone, coordinating tests and treatments, completing forms for home health care and insurance, and maintaining patient flow.

Qualifications

• Minimum required education: Associate's Degree in Nursing.
• Preferred: Bachelor’s Degree in Nursing.
• Current Wisconsin RN license is required.

Responsibilities

• Develop and implement nursing plan of care for patients; communicate plan and document treatment.
• Triage patient concerns by telephone and provide nursing advice per clinic protocols.
• Review and complete health care forms for home health care, referrals and equipment; assist with insurance forms.
• Maintain patient flow, protect confidentiality, and work as part of the clinic team.
• Assist with clinical trials as required; perform other duties as assigned.
