Job summary

Bethany Children’s Health Center in Bethany, OK seeks a Clinic Nurse Navigator to coordinate patient care across the continuum. You’ll act as patient advocate, educator and liaison within the outpatient clinic, supporting a multidisciplinary team to ensure smooth transitions and timely referrals.

The role requires a Bachelor's degree in a health-related field, 3+ years of clinical experience (pediatric preferred) and RN licensure in Oklahoma or a multistate compact.

Qualifications

• Requires RN licensure in Oklahoma or a multistate license (eNLC).
• 3+ years of clinical experience; pediatric experience preferred.
• Bachelor's degree in health-related field.

Responsibilities

• Cultivates and supports primary care and specialty provider co-management with timely communication, inquiry, follow-up and integration of information into the care plan regarding transitions in care and referrals.
• Serve as the contact point, advocate and informational resource for patient families, care team, and sub-specialists.
• Increases continuity of care by managing relationships with ambulatory care providers, transitions in care, referrals, and authorizations.
• Communicates information and participates in satellite clinics, multidisciplinary clinics, and everyday operational functions of outpatient clinic.
• Directly involved with the development and enhancement processes of multidisciplinary specialty clinics with the aim to improve the clinical experience for patients and families.
• Other duties as assigned.
