Job summary

Konza Prairie Community Health Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our integrated care team for full-time outpatient primary care. You will work with providers, behavioral health staff, and other clinicians in a FQHC setting to deliver holistic, evidence-based care.

The role emphasizes whole-person care, patient education, and collaboration within a mission-driven environment.

Qualifications

• Active Kansas LPN license.
• Current BLS certification.
• Strong working knowledge of EMR.
• 1+ year of experience in primary care, ambulatory care, or community health preferred.
• Experience in an FQHC preferred.
• Bilingual (Spanish) preferred.

Responsibilities

• Perform patient assessments, triage, and document medical histories.
• Collaborate with providers and care team members to deliver high-quality primary care.
• Maintain patient records in the EMR.
• Administer medications, immunizations, and treatments to patients of all ages.
• Assist with procedures and coordinate care, including follow-up and referrals.
• Provide patient education and triage phone calls to support ongoing care.

As a vital member of our care team in a Federally Qualified Health Center (FQHC), you will support the delivery of outpatient primary care alongside providers, behavioral health professionals, and fellow clinical staff. What You’ll Do

• Accurately maintain patient records in the electronic medical record (EMR)
• Administer medications, immunizations, and treatments to patients of all ages
• Assist with procedures and coordinate care, including follow-up and referrals
• Provide patient education and triage phone calls to support ongoing care
