Job summary

Konza Prairie Community Health Center is seeking a compassionate LPN to join our integrated care team in a full-time role. You will work alongside providers, behavioral health staff, and clinical teammates in a Federally Qualified Health Center setting to deliver patient-centered, evidence-based care.

You will perform assessments, document histories, manage medications and vaccines, and support care coordination. No nights or weekends; clinics close early on Fridays.

Qualifications

• 1+ year experience in primary care, ambulatory, or community health preferred.
• FQHC experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Perform patient assessments, triage, and document medical histories.
• Collaborate with providers and care team to deliver high-quality primary care.
• Maintain patient records in the EMR.
• Administer medications, immunizations, and treatments to patients of all ages.
• Assist with procedures and coordinate care, including follow-up and referrals.
• Provide patient education and triage phone calls to support ongoing care.

As a vital member of our care team in a Federally Qualified Health Center (FQHC), you will support the delivery of outpatient primary care alongside providers, behavioral health professionals, and fellow clinical staff. What You’ll Do

Requirements

• Active Kansas LPN license
• Current BLS certification
• Strong working knowledge of EMR
• 1+ year of experience in primary care, ambulatory care, or community health preferred
• Experience in a FQHC preferred
• Bilingual (Spanish) preferred
