Job summary

Trinity Health seeks a part-time Licensed Practical Nurse (LPN) to provide direct patient care in outpatient and clinic settings, supporting providers with procedures, education, and care coordination.

The role includes collecting vital signs, preparing patients, administering medications, and documenting results while promoting patient safety, confidentiality, and evidence-based practice.

Qualifications

• Active LPN license in Iowa.
• BLS certification within 3 months of hire.
• Mandatory Reporter abuse training within 3 months of hire.
• Proficient with basic computer skills (Windows, email, MS Office).
• Ability to communicate effectively with patients, visitors and staff.

Responsibilities

• Deliver patient care through preparation, vital signs, and documentation.
• Administer medications, treatments, and immunizations under providers.
• Prepare and organize patient charts and complete required documentation.
• Schedule procedures, labs, imaging, and other tests as needed.
• Coordinate referrals, follow-ups, and insurance pre-certifications.
• Maintain patient confidentiality and adhere to standards and policies.
• Educate patients and families based on assessed needs and discharge plans.
• Assist in training and supporting clinic staff; ensure continuity of care.
• Document care plans and escalate non-routine issues to management.
