Job summary

Community Health Systems, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver essential nursing care in a clinic setting under supervision of licensed practitioners or RNs.

You will perform direct patient care, assist with procedures, administer medications, conduct diagnostic testing, and document in the electronic medical record to support care plans and patient education. You’ll collaborate with healthcare teams, maintain HIPAA compliance, ensure a safe, sanitized clinic environment,

Qualifications

• 0-2 years of clinical or outpatient experience.
• Current LPN or LVN licensure required or in process.
• Basic Life Support (BCLS) certification required.

Responsibilities

• Take patient intake, document symptoms, medical history and vital signs in the EMR.
• Administer medications via oral, inhalation, or injection routes and assist with procedures.
• Conduct diagnostic tests such as EKGs and basic lab work.
• Ensure timely follow-up with patients through portal, calls, and test results.
• Educate patients on treatment plans and preventative care.
• Maintain clean, sanitized clinic environment and proper waste disposal.
