Job summary

CHI Health Clinic is seeking an experienced Licensed Practical Nurse (LPN) to help keep the clinic running smoothly for patients and the clinical team. You’ll manage chart preparation, health history collection, vital signs, injections, wound care, and medication administration according to guidelines.

You’ll collaborate with physicians and staff to prioritize tasks, educate patients and families, and ensure patient confidentiality and quality care in a fast-paced outpatient setting.

Job Summary and Responsibilities

As our Clinic Licensed Practical Nurse (LPN), you'll keep the day running smoothly and efficiently for your patients and clinical team. You'll take the lead on proactive preparation of patient charts; ensuring all information is obtained and up-to-date for those seen in our facility. Every day, you'll manage care for multiple patients, adapting to physician availability on-site, and actively collaborate with your clinical team to prioritize tasks and ensure seamless patient flow throughout the clinic. Your success in this role comes from your enthusiasm to help others, work efficiently, and your empathy for our patients.

Performs general nursing care to patients by following established clinic workflows.

May perform specific nursing care as it relates to specialty of the practice.

Obtain patient consent for care and ensure patient confidentiality.

Obtain and document health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition.

Provide patient care based on practice guidelines, standards of care, and federal/state laws and regulations. Execute prescribed treatments and medical interventions, administer prescribed medications, and monitors and documents treatment progress and patient response. This includes but is not limited to preparing and administering injections, performing routine tests, treating wounds, and changing bandages. Refer to/consult with physicians, other health providers, and community resources to prevent/resolve problems or concerns.

Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.
