Responsibilities

• Processes incoming patient referrals for two clinics using guidelines and clinical judgment.
• Coordinates with patients, internal providers, staff, and external professionals to streamline referrals.
• Manages patient outreach with a standard 3-attempt scheduling protocol.
• Notifies referring providers when patients cannot be reached.
• Fulfills incoming medical record requests following regulatory guidelines.
• Facilitates medical correspondence and administrative tasks for providers.
• Maintains strict quality control and adheres to clinical procedures.
• Audits patient records to identify, investigate, and resolve discrepancies.
• Extracts data and completes electronic case report forms.
• Responds to and resolves queries in a timely fashion.
• Contacts outside providers to obtain subject records.
